How do I remove the default service request template from the portal?

When you are an analyst you have the option to click +, Work item, Service Request, New Service Request. This is a form that has a bogus Review and Manual Activity. Before the Service request can be closed these activities need to be completed. I would like to remove this template from the Portal but I'm unsure how to do this.

Hi Gina
You just need to change the default template in your admin settings. By default the "Default Service Request" template contains two activities, so just determine the custom template name and insert in the settings.

You can hide by scoping it out of the user role, but that becomes tricky to maintain across multiple roles as new templates are introduced.
