History should capture Vote mismatch and be reportable (email notification or Notify app)

Without over complicating the roles, having analysts approve on behalf of someone else is fraught with security risk. Review decisions are captured in history but not a mismatch, whereby Tom (Manager) is assigned as the Reviewer, however John (stable boy) votes instead. As a Process/Service Owner or Tom, I would want to know that the stable boy has approved my RA to ask why.

Additionally, it should capture in history whether someone has changed a predefined Reviewer. These things may be investigated down the track, but there should be controls to prevent these situations at all, or have a more immediate method for notifying people.
