Change Icon/Update Action Log when Incident is Reactivated

If an end user reactivates their incident, it would be nice to show the incident with a different icon (to make it stand out for the analyst.)  In addition, it would be helpful to add the reactivation event to the Action Log on the incident form.

Even more helpful would be a prompt to add a comment (required?) when and end user reactivates their incident to help the analyst that was assigned to it.
