What questions do you use for the Change Risk Calculator?

I'm not sure about you, but I love the Change Risk Calculator as a super simple app that saves me having to enter all the same questions and answers over and over again and helping me remember what questions to put in so the change manager doesn't com chasing me for more details a a later date.

But while writing some training sessions on this today I thought, "What are the best questions to ask?"

The answer to this is going to be different for every organization, but I am curious what people have as their questions in their setup for Change Risk Calculator.

    2. Is a reboot required?
    3. Is this change affecting a Business Critical app?
    4. Is end user training needed?
    5. Is more than one site affected?

    My questions are below, they may seem odd to some of you but we work on 4 weekly periods not monthly, we also have a lot of remote sites.

    Will this affect sites other than Head Office?

    Does this change Impact more than 30 employees?

    Will training be required for Employees when this change is released?

    Will implementing this change affect the availability of the Live Environment?

    Is this change affecting a Business Critical Service?

    Is this change close to a Period End?
