On initial install survey pages won't render.

Just installed the Survey App and am having issues.  Pages show on the left side navigation and appear to load without error.  However they render blank in the content section.
We are using Windows 2012R2, SCSM 2012R2, Cireson portal 8.3.1.2012 and Survey 7.6.0.13.  Console License app shows license.  Workflows are executing without error.  No errors in Cache builder, No errors in event log.  Portal works perfectly.

new Advanced Operator setup, Groups configured per setup instructions.

Any Ideas?

    My bad.  Just found a typo in the Directory name.

    If you open the F12/Dev tools, what do you see in the console?
