Activity in view with error

I had some code written that used the API to add activities to a request when certain conditions applied.  It looks like that has caused an issue with a particular CR in our system.  There's an activity that is in my My Work view, but if I open the CR itself, the activity is not present.  Is there any way that I can purge this activity from the system?

I also cannot find this activity within Service Manager itself if that helps.
