Request form does not match prompt configured in SCSM

Hi,

I am trying to figure out why our request form is missing a step and cannot seem to track down what the issue is. In SCSM is it configured with four questions but only three are displayed in our ITSM portal. I have attached pictures to show the difference. Missing the "Please specify option".

    Hi Brandon,

    If you edit the RO with the Advanced Editor (don't double click but see option on the right hand side) you will see a layout section. That might be where a conditional visibility is configured.
