Change Requests - Review Activities getting stuck in Pending

We have a series of approvals inside a PA and we occasionally see that staff create changes via the Cireson Portal and click submit, but for whatever reason, the activities get stuck in Pending and never change. Our workaround has been to change the status to On Hold and then back to In Progress which gives the activities a kick.

Has anyone experienced this issue with Service Manager or the Cireson Portal? Anyone find a cause?
